Direct anterior approach (DAA) hip replacement: a clear guide

Now: DAA hip replacement is part of current practice when the approach is judged suitable.

Families often hear “front approach” or “DAA” when a loved one needs a hip replacement. This page explains Direct Anterior Approach hip care with Dr Ravi Vattipalli at Care Hospitals Arilova: what it is, how training in Genk shaped the pathway, what same-day walking can mean when the team clears it, and where honest limits sit.

What DAA is

Direct Anterior Approach is a way of reaching the hip joint from the front of the hip. Suitability depends on anatomy, prior surgery, bone quality, medical fitness and surgical judgement, not on a marketing label. Some patients may be better served by another approach.

DAA is part of current practice when the approach is judged appropriate. It is not the same as robotic hip surgery. Robotic hip replacement remains a future roadmap item and is not offered as a live service today.

A teaching day in theatre in Genk, Belgium

Dr Ravi Chandra Vattipalli spent a day in theatre in Genk, Belgium, focused on Direct Anterior Approach hip replacement. The list was built as a teaching day: a simple primary hip, a complex post-traumatic reconstruction, a revision and other cases in between, under different degrees of difficulty.

What stayed was not only the technique. Soft-tissue releases were taught step by step, so that the reason sat next to the method: when to proceed, and when to hold back. Training abroad does not guarantee a result for any one patient.

What changed in the Care Hospitals pathway

The practical outcome of that training is a pathway change at Care Hospitals, Visakhapatnam: the team has been able to incorporate same-day mobilisation into hip surgeries as well as knee surgeries.

When it happens, it follows the same ERAS discipline used for early walking elsewhere in the practice. The anaesthetist clears the patient to stand, the therapist manages the walker, and the pathway can say “not yet” when the patient is not ready. Same-day walking after a direct anterior hip replacement is a planned, safety-gated step. It is not a promise for every case.

  • Hip recovery plans differ by approach, implant and the person’s health.
  • The robotic knee ERAS pathway should not be assumed to apply unchanged to every hip case.
  • At consult, families can ask what this hip pathway looks like for the person in their care.

“Knowledge travels. Patients gain.”

What a hip consultation reviews

  • Pain, walking difficulty, sleep and daily care
  • Previous surgery, imaging, anatomy and bone quality
  • Medical fitness for anaesthesia and surgery
  • Whether DAA or another approach is safer for the specific hip
  • Weight-bearing, hospital course, risks and home support

Risks and honest limits

Every hip replacement carries risks that should be discussed in person, including infection, blood clots, fracture, nerve or soft-tissue injury, dislocation, implant wear or loosening over time, anaesthesia risks, and the chance that recovery is slower than hoped.

DAA hip replacement FAQs

What is DAA hip replacement?

Direct Anterior Approach is a way of reaching the hip joint from the front of the hip. Suitability depends on anatomy, prior surgery, bone quality, medical fitness, and surgical judgement.

Who might be suitable for DAA?

People needing hip replacement for arthritis or selected complex or revision situations may be candidates after assessment. Some hips are safer with another approach, and that decision is made at consult.

Is robotic hip surgery available now?

No. DAA hip replacement is part of current practice. Robotic hip replacement is on the roadmap next and is not offered as a live service yet.

Can patients walk the same day after DAA hip surgery?

Same-day mobilisation can be part of the hip ERAS pathway when the team clears the patient, as a short, supported walk. It is not promised for every patient; ask what applies in your case.
